
Application Requirements
All Loans with items of value over $150,000.00 require:
- Completed and signed commercial loan application
- Completed and signed personal financial statement
- Last three years of signed and dated business tax returns. If you have not filed for the latest tax year for your business, please also provide a copy of your IRS form 7004 filing extension request.
- Last three complete fiscal years of CPA prepared financial statements and year-to-date management prepared financial statements for the current fiscal year.
- A current accounts receivable and accounts payable aging report.
- An accounts receivable and accounts payable aging report dated as of the date of the interim year-to-date management prepared financial statement for the current fiscal year.
- Last two years of signed and dated personal tax returns or copy of W2(s) or 1099(s) if you have not filed for the last tax year. If you have not filed, you will also need to provide a copy of your IRS form 4868 filing extension request.
- Description of the furniture, fixtures or equipment being purchased. You may use the dealer invoice for this purpose or provide:
1. Complete schedule of equipment including serial numbers, description, model number and number of units;
2. Copy of a sale contract reflecting the contract price and seller’s name.
- Resumes of all corporate officers and shareholders.
- Copy of your business’s marketing materials or web address.
- Names and relation to you or your business of any subsidiaries, affiliates or related business enterprises.
- If a corporation: Copy of the business organizational documents including your Articles of Incorporation, Bylaws and current certificate of good standing from the state in which the business is organized.
- If a limited liability corporation: please provide a copy of the Articles of Organization, Operating Agreement and current certificate of good standing from the state in which the business is organized.
- If a Partnership: Copy of partnership agreement. If a limited partnership, a copy of the LP-1 or LP-2 (if applicable) must also be submitted.
- If a sole proprietorship: Copy of the proprietorship’s tax ID number.
